Letter of Recommendation Request Forms
All colleges will want a letter of recommendation from your school counselor, and at least one from an academic teacher. Scholarship applications may also require recommendations. You should choose a teacher who knows you well enough to write positive things about you, even if you didn't get a superior grade in that class. Give the teacher plenty of time to write the letter and supply the earliest due date. Junior year teachers may be the best choice since they have had you recently for an entire year. Senior year teachers may not know you well enough, unless they had you in the past. Coaches and club advisors might provide good additional letters, but not instead of academic teachers.
In order for the counselor and the teachers to have some information to assist with the letters, please use the following request forms.
Please note: It would be appropriate to send the teacher(s) a thank you note after the letter is written.
